How do you document a difficult conversation without it reading like a legal threat?
I write up every ER conversation for the file, but my drafts always come out sounding harsher than the actual conversation felt. Anyone have a format that stays factual without feeling adversarial?

Write it in past tense, third person, facts only — no adjectives about tone or attitude. 'Raised voice' not 'became aggressive.'
